III. Required Customer Information

- Full legal name

- Residential or business street address

- Date of birth (for individuals)

- Taxpayer Identification Number (SSN/EIN) or foreign equivalent

- Phone number and email

- For entities: Certified Articles of Incorporation, Business License, Authorized signatory’s ID

IV. Acceptable Forms of Identification

- U.S. driver’s license

- State-issued identification card

- U.S. or foreign passport

- U.S. military ID

- For entities: business license and corporate documents

All customers must complete the USA PATRIOT Act Compliance Form with supporting ID.

VII. Payment Thresholds and Reporting

- Cash accepted up to $10,000 per day per customer; CTR required for >$10,000 or structured transactions.

- Suspicious transactions of $5,000+ will be reviewed for SAR filing.

All customer identification records and Patriot Act forms are retained for five (5) years after the transaction date.
